Post-Surgical Rehabilitation: What to Expect After Orthopedic Surgery
Orthopedic surgery is often just the beginning of the recovery process. Whether you're having a knee replacement, rotator cuff repair, ACL reconstruction, or spinal surgery, the rehabilitation that follows is equally — if not more — important than the surgery itself.
Why Post-Surgical Rehab Is Essential
Surgery repairs damaged structures, but it also creates trauma to surrounding tissues. Swelling, muscle inhibition, scar tissue formation, and loss of range of motion are all normal consequences of surgery. Physical therapy addresses each of these issues systematically to optimize your recovery.
The General Timeline
While every surgery and patient is different, here's a general framework:
Week 1-2: Protection Phase
The focus is on managing pain and swelling, protecting the surgical repair, and beginning gentle range-of-motion exercises. You'll likely use ice, elevation, and compression regularly. Your physical therapist will teach you safe movement patterns for daily activities.
Week 2-6: Early Mobility Phase
As healing progresses, we gradually increase range of motion and begin light strengthening exercises. Scar tissue management through gentle massage helps prevent adhesions. Gait training may begin for lower extremity surgeries.
Week 6-12: Strengthening Phase
This is where significant progress happens. Progressive resistance training rebuilds muscle strength and endurance. Functional exercises simulate real-world activities like climbing stairs, reaching overhead, or walking on uneven surfaces.
Week 12+: Return to Activity
The final phase focuses on sport-specific or job-specific training, power development, and full return to previous activity levels. Objective testing ensures you're ready.
Pre-Surgical Rehabilitation (Prehab)
Research shows that patients who participate in physical therapy before surgery — called "prehab" — recover faster and have better outcomes. Prehab focuses on maximizing strength and range of motion before the procedure, giving you a head start on recovery.
What to Look for in a Post-Surgical PT
Choose a physical therapy clinic that:
- Has experience with your specific surgery
- Communicates directly with your surgeon
- Provides one-on-one treatment sessions
- Uses objective outcome measures to track progress
- Creates individualized treatment plans
